For Patients

For patients and families

What Reliance means for you, and the choices you have.

Reliance eHealth Collaborative helps the people caring for you work from the same, up-to-date information. Here is what that means for you.

What Reliance means for you

When you see a doctor, visit urgent care, or are admitted to a hospital, each visit creates information about your health. Reliance securely connects those providers, so your care team can see a fuller picture of your care instead of starting over each time. That means safer care, fewer repeated tests, and less time spent re-explaining your history.

Is my information private?

Yes. Your information is protected under HIPAA and shared only with authorized providers involved in your care. Reliance is a community-governed nonprofit, and access rules are set by Oregon's healthcare community, not a vendor. See our privacy and security questions for more.

Your choice to opt out

You can choose whether your information is shared through the Reliance network. To opt out, complete the opt-out form and return it as described on the form. If you change your mind later, an opt-out cancellation form restores sharing. Forms are available in English and Spanish.

Patient access

You have the right to access your own health information, and Reliance offers more than one way to do that. One option we provide is Selfie, a way for you to access your information directly. Scan the code to get started.
