When a patient leaves one provider, their care does not stop. They see specialists, visit urgent care, and are admitted across town, and each encounter creates information the next provider may never see. Reliance eHealth Collaborative exists to close that gap, so the people caring for a patient work from the same, current information.
Who we are
Reliance is Oregon's community-based, nonprofit Health Information Exchange, connecting providers across Oregon and Washington since 2012. We are the shared infrastructure that brings a patient's clinical record together from every connected organization, hospitals, health plans, primary care, behavioral health, oral health, and public health, into one near-real-time view. As a 501(c)(3) nonprofit governed by Oregon's own healthcare community, our value rests on one principle: every connection makes care better for every patient in the community.
